Transition Out of BSW

Wherever your next steps take you, we wish you the best! Before you go, here are a few things you need to know and do.

  • The Basics

    When you know your last day of work, notify your manager and ensure your contact information (phone number, email and address) is current in PeoplePlace. You’ll have the opportunity to give feedback about your experience at BSW in an exit interview survey.

What To Do

Once you’ve picked your retirement date, follow these steps for a smooth transition:

 

Let your manager know

Provide your manager written notice of your resignation at least two weeks before your last day. If you’re a manager or above, you’re expected to give at least four weeks’ notice. Be sure to indicate the reason for your departure and last day of work.

Transition your work

Work with your team members and manager to ensure a smooth transition of your work. Keep in mind that your BSW email and network access will end on your last day of work.

Update your contact information

Be sure to update your contact information in PeoplePlace > About Me so you receive information on benefits continuation opportunities as well as your W-2.

If you have additional questions, submit a ticket to PeoplePlace (log in required). 

Empty out your workspace 

Remove any personal possessions from your workspace. If you have a workspace at home, you will need to return any sensitive information to BSW. Remove any work-related virtual files on your personal computer or other personal devices.

Return company materials

On your last day, return your ID badge and other company-provided materials—such as your keys, cell phone and computer—to your manager.

Complete your exit interview survey

You will be sent an email with an invitation to complete an exit interview survey once your manager has submitted your resignation. Complete this survey ahead of your last day of work.

If eligible, you will receive a COBRA notice two to three weeks following the last day of your employment. You will have 60 days from the date of the notice to elect COBRA.

View details around the the continuation and/or timing of various benefits. Note: Applies to eligible general staff, APP's, directors and CRNA's.

Employment Verification

For employment verification for commercial entities such as a mortgage loan, auto finance, credit card, job offer, apartment lease, etc., employers can visit theworknumber.com or calll 1.800.367.5690.

For employment verification for Social Services entities such as Medicaid, SNAP, TANF, subsidized housing, etc., employers can visit theworknumber.com or call 1.800.660.3399.

If there is a discrepancy with information received from The Work Number, a verification form can be faxed to PeoplePlace at​ 254.899.3770 to be completed. PeoplePlace will then review and complete the form within 72 hours. The employer/employee would then need to contact PeoplePlace at 844.417.5223 to notify Baylor Scott & White that the information is incorrect so that the issue can be fully investigated.
