Life insurance and accidental death and dismemberment (AD&D) insurance protect your family financially if you die or are seriously injured.

We provide you with basic life and AD&D coverage through Cigna at no cost.
You can purchase additional coverage for yourself, spouse or children at the premiums below.

Note: Other coverage levels are available for certain executive and physician roles and can be viewed during enrollment by visiting PeoplePlace.

Basic and Supplemental Life

Basic Life Insurance

Provided to all benefits-eligible employees at no cost.

Pays a benefit of 1x your annual salary (minimum $20,000; maximum $500,000)
There's no medical underwriting and coverage begins on your first day of employment.

Supplemental Life Insurance

Available to all benefits-eligible employees at your own cost. You can purchase additional coverage for you, your spouse and/or child(ren).

You: Benefit amount up to 7x salary | maximum $2 million
Spouse: Up to $250,000 | but not more than your benefit amount
Child: $10,000

_{Evidence of insurability is required only if you increase your current employee coverage by more than 3x your salary or enroll your spouse in coverage above $75,000.}
