Summary of Benefits and Coverage (SBC) is an easy-to-understand summary about each medical plan option offered by Baylor Scott & White Health. This summary makes it easy for you to compare coverage between all three network Tiers and how different services and procedures are covered.
This booklet contains Summary Plan Descriptions (SPDs) that describe how the Baylor Scott & White Health employee benefit plans (the plans) work. Each section has important details about specific plans. This section provides important information on eligibility, enrollment and paying for coverage.
If you or your children are eligible for Medicaid or CHIP—the Children’s Health Insurance Program—and you’re eligible for health coverage from your employer, your state may have apremium assistance program that can help pay for coverage using funds from their Medicaid or CHIP programs. If you or your children aren’t eligible for Medicaid or CHIP, you won’t be eligible for these premium assistance programs but you may be able to buy individual insurance coverage through the Health Insurance Marketplace.
